Firstly the size of the heli depends on the size and wieght of the camera you can use. You then have to take into concideration the power sorce and the transmitter wieght if you are carrying a transmitting pack. of course if this is the option you go with then you will have to have the receiver settup and portable recording device.
As you can imagin it all goes to add wieght and cost. If you have a small camera allready then you can install it useing velcro or rubber bands or double sided foam pads. It really is a question with many answers subject to the type of helicopter and the type of system you want to use.
You will have to pay very close attention to the C of G (center of Gravity) on the heli and make sure that you ballance it as best you can. you will want the heli to be as stable as it can be with the camera system on board. I would concentrate on the flying of the helicopter and get good at nose in hovering before I would concider going into camera heli's. The reason for this statement is that it is one thing concentrating on flying a heli, but if your going to split your attention between heli and what you want to film then you need to be good enough to cope.
